Medical Records Transfer

An administrative fee of $10.00 applies for transfer of medical records.

Lost scripts, referrals and requests for additional documentation

A $20.00 fee will apply for re-issuing of lost referrals or scripts. Our doctors reserves the right to refuse a script request if a consultation is required for safe prescribing. No Medicare rebates apply.

Request for medical certificates after your appointment has concluded and you have left are subject to a $20.00 fee. No Medicare rebates apply.

All fees are due upfront.
