New Patients

We ask all new patients book a 30 minute appointment for their first appointment. This will give our doctor time to discuss and document your medical history.

 

You can book an appointment online through Automed.

 

We are a private billing practice. Please refer to our fees page for further information.

 

Please arrive 10 minutes prior to your appointment time to complete the New Patient Registration form, if you have not done so online. Alternatively, you can complete the form here. Please email completed form to info@princehenrymedical.com.au. Please bring your medicare card and any concession cards to your appointment.

Newborns

 

We ask that a 45 minute appointment be booked for newborns for their 6 week check and immunisation. Please make an appointment via Automed or call reception on 9921 1708 to make this appointment as we will also need to make an appointment with the practice nurse.
